Mortgage Appointment Checklist
1. Most recent one months’ income verification (check stubs).
Example - If you are paid bi-weekly, bring your last 2 check
stubs. If you are paid weekly, bring your last 4 check stubs.
2. Last two years W-2 Statements
for each place of employment.
3. If self-employed or commission
based income-need last two years tax returns including W-2’s.
4. Last 2 months’ bank statements
on all checking, savings and money market accounts.
5. Asset Verification such as, 401K, cash value from life
insurance policies, stocks, bonds, annuity or certificate of
deposits. Please include the most recent monthly, quarterly
or yearly statement.
6. Homeowners Insurance
declaration page, which has the agent's name and address,
coverage amount, coverage period and premium amount.
7. If refinancing any first or
second mortgages, need the most recent statement for each
mortgage. This statement should include mortgage balance,
company name, address and telephone. If you only have a payment
book, please bring it in place of the statement.
Bringing in the above information, along with the application
filled out completely, speeds up both the approval process and
loan closing.

Please provide the following with your application so that we may
process your request more quickly:
1. Completed Application
2. Copy of your last 2 pay stubs or W-2’s – If self employed, last 2

3. Copy of Insurance Binder or agent's name & phone number

Please provide the following with your application so that
we may process your request more quickly:
1. Completed Application
2. Copy of your last 2 pay stubs or W-2’s – If self employed,
last 2 years tax returns
3. Copy of Home Owners Insurance Coverage or agent's name & phone
number
The typical Home Equity Loan request takes approximately 10 days
to process.
